| Become a Fellow of the ASCP Being a Fellow of the American Society of Consultant Pharmacists (FASCP) signifies that you have achieved a balance of experience that goes well beyond regular pharmacy practice. To be recognized as a Fellow in ASCP, an Active Member (pharmacist member) must earn at least 125 Fellowship points every five years. These points are earned in 12 different categories as shown on the application. If you are interested in being recognized as a professional who has met the highest standards in the field of consultant and senior care pharmacy, please review the 12 categories for Fellowship status. (You can download a copy of this application in Acrobat PDF format). As you earn points within the different categories, fill them in. There is a limit to the number of points one can obtain in each category. Thus, qualified Fellows must demonstrate a balance between practice duties, professional experience, and community involvement. As soon as you have earned a total of 125 points, complete the application, and send it along with the application fee (you may use the form below and pay online by credit card). Remember, the 125 points must be earned within the five years immediately proceeding the date of application, except for the lifetime achievement category Your membership category in ASCP must be Active Member (pharmacist member). Although ASCP will not require a detailed list of all CE credits (due to the space provided), please keep a copy of all "points" and CE credits you use to qualify for the FASCP designation.
