Managing warfarin in the elderly can be like following a bouncing ball. Balancing the risk of bleeding due to falls against the need for anticoagulation can be tricky.  Come learn more about a pharmacist-managed anticoagulation service for geriatric patients living in the community and assisted living facilities.  We will share the policies and procedures that started our service within a PACE (Program of All-inclusive Care for the Elderly) program as well as the day-to-day activities of identifying new patients, scheduling appointments, using point-of-care testing devices, monitoring therapy, and adjusting doses.  Through case scenarios we will look at the pharmacokinetics of warfarin we frequently observe, current practice guidelines, drug interactions (or lack thereof), and how we manage INRs that inexplicably rise and fall.
